Supportive Housing Overview

Serving individuals in recovery from mental health and substance use disorders, including those living below the federal poverty level.

Our Supportive Housing programs offer customized services based on individual need and include evaluation and treatment; family education and support; coordination with other service providers; and case management.

Independent Living Program: Serves 182 individuals

  • Provides case management and housing support services to assist individuals living independently in their own apartment.

  • Providing support services and housing vouchers for scattered site apartments throughout Monroe County.

  • Funded through the NYS Office of Mental Health (OMH).

Permanent Supportive Housing: Serves 20 individuals

  • This program is based on the “housing first” model.

  • Providing housing and support to individuals with substance use disorders, who have had a high utilization of acute care services.

  • Funded through the NYS Office of Addiction Services & Supports (OASAS).

HOPE Program: Serves 40 individuals

  • Providing support to individuals who have transitioned from inpatient psychiatric units to the community.

  • Funded through the NYS Office of Mental Health (OMH).

Choices Program: Supports 3 young adults

  • Supports those between the ages of 18 and 25 years old

  • Individuals who have recently transitioned out of residential youth services.

  • Funded through the NYS Office of Mental Health (OMH)

Canal Commons:

  • 37 units for people with a primary diagnosis of a mental health disorder

  • 25 units for people with a substance use disorder

  • 8 units for people who are experiencing chronic homelessness

  • This site offers 53 affordable housing units for the Rochester community (not affiliated with East House).
