If your inquiry is urgent please feel free to call 585-238-4800 and ask to speak with someone in the admissions department.
Individuals interested in housing with Office of Addiction Services and Support (OASAS) please use the link to access the Monroe County OASAS application. https://www.monroecounty.gov/mh-addictionsvcs.
Individuals interested in housing with Office of Mental Health (OMH) please use the link to access the SPOA application. https://www.monroecounty.gov/mhadult-spoa.
Canal Commons:
ESSHI SMI: Please complete the Severe Mental Illness Empire State Supportive Housing Initiative Services application and submit to the Monroe County SPOA.
ESSHI SUD: For this program, a Canal Commons Referral Form, Canal Commons Application Package, and ESSHI 2 Homeless Verification Form must be completed. Please e-mail to admissions@easthouse.org.
ESSHI CH: Coordinate with Homeless Continuum of Care providers regarding criteria. For this program Canal Commons Referral Form, Canal Commons Application Package, ESSHI 2 Homeless Verification Form and the Disability Determination Form must be completed.
Affinity Place Short-Term Respite:
Feel like you may be experiencing a mental health crisis? Need a safe place to recover? The hospital isn’t your only option. Affinity Place is a no-fee, short-term, bed and breakfast style housing option staffed by people with lived experience.
Affinity Place is available to residents of Monroe, Genesee, Livingston, Orleans, Wayne, and Wyoming Counties for up to 28 days. Affinity Place doesn’t offer immediate placement. You must call to inquire about getting on the waitlist.
After your stay at Affinity Place, our East House staff will support you in your search for any future housing opportunities.
If you are 18 years or older, have a diagnosis of a serious mental illness and are experiencing a crisis you may be eligible for a short-term stay.
