CORPORATE COMPLIANCE PROGRAM
East House has established a Corporate Compliance Program to demonstrate its commitment to the highest standards of ethical conduct, accountability, and compliance in all areas of its operations. The Program is intended to promote ethical behavior, support quality service delivery, and reduce the risk of fraud, waste, abuse, unethical conduct, and violations of applicable federal and New York State laws and regulations.
The Corporate Compliance Program reflects East House’s mission, vision, and organizational values, and reinforces the Organization’s commitment to the individuals it serves, those who work on its behalf, and the broader community. It defines East House’s expectations regarding ethical behavior, transparency, and compliance, and outlines the framework used to encourage the reporting of concerns and ensure that issues are addressed promptly and appropriately.
This Program applies to East House board members, employees, volunteers, interns, contractors, vendors, and other individuals or entities acting on behalf of the Organization. East House’s commitment to ethical and lawful practices is essential to maintaining public trust and delivering services with integrity, dignity, and respect.
STANDARDS OF CONDUCT
East House has established written Standards of Conduct to promote ethical behavior, integrity, and accountability in all aspects of its operations. The Standards of Conduct reflect the Organization’s commitment to complying with applicable federal and New York State laws and regulations, and to conducting its business in a manner that is consistent with its mission, values, and responsibility to the individuals it serves.
The Standards of Conduct provide guidance to all individuals associated with East House regarding expected standards of behavior, professional responsibility, and ethical decision-making. They address key areas such as compliance with laws and regulations, confidentiality and privacy, professional boundaries, fair and respectful treatment of others, accurate documentation, and the obligation to report suspected misconduct or noncompliance without fear of retaliation.
The Standards of Conduct apply to East House board members, employees, volunteers, interns, contractors, vendors, and other individuals or entities acting on behalf of the Organization. Adherence to these Standards is essential to maintaining public trust, supporting quality service delivery, and ensuring that East House operates with integrity, transparency, and respect.
MANDATORY & CONFIDENTIAL REPORTING INFORMATION
East House is committed to maintaining the highest standards of ethical conduct, accountability, and safety in the delivery of services and in all business practices. As part of this commitment, East House provides clear and accessible mechanisms for individuals to report concerns, suspected misconduct, or potential violations of law, policy, or ethical standards.
East House encourages the reporting of concerns related to matters such as fraud or abuse, confidentiality or privacy violations, harassment or discrimination, health and safety issues, and other serious compliance or ethics concerns. Reports may be made directly to supervisory staff or to the Chief Ethics, Compliance & Quality Officer. East House also maintains a confidential Compliance Hotline that is available 24 hours per day, 7 days per week, and allows for anonymous reporting.
East House strictly prohibits retaliation against any individual who reports a concern in good faith or participates in a compliance or ethics review. All reports are taken seriously and are reviewed and addressed in accordance with East House’s Corporate Compliance Program and related policies.
NOTICE OF PRIVACY PRACTICES
East House is committed to protecting the privacy and confidentiality of the health information we create and maintain in the course of providing services. This Notice of Privacy Practices describes how East House may use and disclose protected health information, the safeguards we maintain to protect it, and the rights individuals have regarding their health information under applicable federal and state laws.
The Notice explains how information may be used for purposes such as treatment, payment, and health care operations, as well as other uses and disclosures permitted or required by law. It also describes your rights, including how to request access to your records, request amendments, request restrictions, request confidential communications, and how to file a complaint if you believe your privacy rights have been violated.
The Notice applies to East House employees, workforce members, and authorized contractors who may create, access, or use protected health information in connection with providing services or supporting operations. When this Notice refers to “you,” it also includes your personal representative, as permitted by law.
