Community-Based Services Overview

Our vision is that East House will become the behavioral health provider of choice for quality supportive housing, including a full range of recovery support services, in the Greater Rochester area.

Recovery Support Services

East House’s Recovery Support Services (RSS) are built on the dimensions of wellness with a focus on environmental, financial, intellectual and occupational, physical, spiritual, social and emotional. Our goal is to help bring our client’s lives into balance. These services consist of housing, counseling, job coaching, adult basic education, case management, skills training, respite care and family outreach.

East House works with a network of community providers to assist our clients in achieving success. Networks include psychiatry, medicine, education, job rehabilitation, mental health advocacy and child services.

  • This collaboration with network providers expands access to East House’s multidimensional programming, supporting individuals experiencing mental health and/or substance use disorders in achieving success and living purposeful lives within their communities.

Education & Employment Services:

  • These services are designed to help individuals develop necessary skills, experience and confidence to increase their employment and volunteer opportunities.

  • Services include: GED preparation; computer skills training; cognitive remediation; vocational assessment and counseling; job readiness training; referrals to schools and skills training programs; job development placement and retention assistance.

  • Funded through the NYS Office of Mental Health (OMH), NYS Office of Addiction Services and Supports (OASAS) and private funding streams.

Home and Community-Based Services (HCBS):

  • HCBS plays a key role in East House’s mission as we work to empower individuals in recovery to have an optimal quality of life.

  • HCBS provides services that allow symptom reduction which encourage treatment, education, employment, overall health, stable housing and general well-being.

  • East House provides the following HCBS services: educational support services; pre-vocational services and empowerment services (peer support).

CORE Services - Peer Empowerment:

  • East House currently employs more than 15 peer support specialists, across our agency.

  • This program empowers individuals to seek and obtain their NYS Peer Certification as well as continue their education and seeking employment growth opportunities.

  • Peers working with our clients have lived experience with mental health and or substance use disorders.

Connections (RSS):

  • A social, health and wellness initiative run by peers (individuals with lived experience) for East House clients.

  • Connections strives to achieve independence in the following areas: financial; emotional; spiritual; social; occupational; physical; intellectual and environmental.

  • Programs can be on a 1:1 basis or in a group setting to promote social wellness among cohorts.

ACCES-VR Services:

  • Through ACCES-VR, East House provides standardized testing to access strengths, barriers, learning styles, interests, and career decision-making. In addition, we offer comprehensive job readiness and placement services, including resume and application support, addressing employment barriers, computer skills assessment, job search support, and registration with Rochester Works.

  • Once a client is employed, East House staff provide weekly follow-up and retention support for up to 90 days.

Health Homes Care Management:

  • Our team works to assist individuals with physical and mental health diagnoses to live their most independent life possible.

  • We work with you to develop a plan of care to address these concerns, including advocacy, linkages, and additional resources to support the individual in meeting their needs.
